Annual check–ups are available with the nurse/health care assistant on a range of diseases which include:
- Asthma
- Diabetes
- COPD
- Heart Disease
- Epilepsy
- Hypothyroidism
- Hypertension
- Stroke & TIA
- Rheumatoid Arthritis
- Peripheral Arterial Disease
- Chronic Kidney Disease
A letter will be sent out annually informing patients that there annual review appointment is due. Patients are requested to make an appointment for this.
